A Strategic Healthcare Leader Redefining Care

Silvana Fischman

Healthcare Consultant Founder and CEO

Chai Class Consulting, LLC

When healthcare organizations struggle to improve patient outcomes while controlling costs, transformation often feels impossible, but that is precisely where Silvana Fischman holds expertise and emerges as a visionary leader. As Founder and CEO of Chai Class Consulting, she partners with startups and established healthcare organizations to streamline operations, implement value-based care programs, and deliver measurable results in both clinical and business outcomes. Her journey spans executive roles at ChenMed, where she led initiatives to improve documentation accuracy; at Duo Health, where she optimized multi-state operations; and at Marine Polymer Technologies, where she strengthened organizational performance. Today, Silvana applies this expertise to help leaders turn vision into action, driving operational excellence, sustainable growth, and human-centered care. During an exclusive conversation with TradeFlock, she shared her journey, lessons, and vision for the future of healthcare leadership.

What shaped your leadership ideology as you transitioned from healthcare practitioner to consulting founder?

The transition from being a healthcare practitioner to founding Chai Class Consulting was never easy, and I still remember how much uncertainty I faced. Leaving dentistry in Mexico City, moving to the United States, and raising a young child while navigating financial struggles forced me to grow in ways I could not have imagined. At one point, excellence became my only choice because I had to make every step count for my family and myself. Each chapter of my career, from medical devices to risk adjustment, value-based care, and ultimately launching my consulting firm, taught me flexibility through uncertainty, empathy through hardship, and perseverance through reinvention. Mentors and family showed me that leadership is about connection, not control. I now see that leadership begins with embracing every lesson. “I did not plan every turn, but I embraced every lesson. That is where true leadership began.”

What were the toughest leadership decisions in building your consulting firm, and what did they teach you?

The hardest part of building Chai Class Consulting was not the business itself but finding the courage to go it alone. I initially started with colleagues I really admire, and we share vision and strengths. Yet over time, personal responsibilities and differing priorities made it clear we needed to move in a different direction. Splitting from that partnership was painful, but it was extremely necessary to continue our wonderful friendship. I still remember sitting alone, wondering if I could really do this, and honestly, I felt almost lost, yet the weight of uncertainty kept me up. That fear became fuel and taught me to trust my instincts, act decisively, and move forward even when the path wasn’t clear. Leadership, I realized, was never just about guiding others—it’s about guiding yourself with the same empathy, consistency, and determination. Going solo became a recalibration, a test of resilience, and a reminder that leadership begins when you believe in yourself.

How do you inspire leaders and organizations to embrace transformation in a complex industry?

Transformation in healthcare is never just about technology or workflows. It is about changing how people relate to change, which can feel uncomfortable in long-standing systems. I start by listening closely, trying to understand frustrations, fears, and perceptions of the current state. Resistance is rarely about the change itself but about uncertainty. I start by explaining the why rather than the what to a team, and seeing their trust slowly build feels incredibly rewarding. Not everyone will align immediately, and honestly, addressing misalignment requires courage and care. Misalignment is not failure; it often shows where strengths are best applied. At Chai Class Consulting, transparency, celebrating small wins, and co-creating solutions help build momentum. Transformation succeeds when people feel heard, supported, and trusted to evolve, and it endures when teams are aligned in purpose and truly believe in the destination.

"Leaders must see AI as an ally, freeing them to focus on what only humans can do: listen, comfort, inspire, and guide."

What initiatives reflect your philosophy of “people-first transformation,” and how do you measure impact?

People-first transformation only works when listening turns into action. Every initiative I have led, whether in clinical operations, CDI, or value-based care redesign, has centered on one principle: people drive performance. At Chai Class Consulting, we measure impact not just through financial outcomes but through the behavioral and cultural shifts that sustain them. I still remember conversations with team members where small feedback sparked big change, and those moments reinforced why listening matters. KPIs guide decisions on training, development, or structural adjustments, and aligning team members who are not aligned is stewardship, not failure. Feedback loops, pulse checks, and one-on-one conversations help me connect with individuals, recognizing their professional contributions and personal well-being. Acting on feedback transparently builds trust. Combining analytics with empathy consistently improves engagement, efficiency, and retention, turning transformation from a concept into a culture where people understand their value and contribution.

How do you see healthcare leadership evolving with AI and a focus on mental health?

The future of healthcare leadership depends on balancing innovation with humanity. Artificial intelligence can improve efficiency, streamline communication, and provide better decisionmaking tools, but honestly, it cannot replace human warmth, empathy, or connection. “Leaders must see AI as an ally, freeing them to focus on what only humans can do: listen, comfort, inspire, and guide.” At the same time, as conversations about mental health and workforce well-being grow, leadership must value how people feel alongside how they perform. I still believe that healthcare is about people helping people. Technology can make us work smarter, but trust, camaraderie, and human connection drive real progress. Leaders who combine innovation with compassion and efficiency create environments where employees and patients feel supported, valued, and inspired, which I believe is the hallmark of the next generation of healthcare leadership.
